Healthcare Boards

A health professional regulatory board investigating an individual regulated by the board may request from the Authority controlled substance information pertaining to the member.  ORS 431.966

The board’s Executive Director or his/her designee must certify that the information is necessary to the board’s investigation.  OAR 410-121-4020

Board Information Request Procedure

Boards must perform the following steps to request information from the PDMP:

  1. Click on the Board Info Request link located on the left menu.
  2. Complete the fillable Board Information Request Form.
  3. Print a hard copy of the completed Board Information Request Form.
  4. Have the Executive Director or his/her designee sign and date the Board Information Request Form.
  5. Submit the signed Board Information Request Form along with a summary of the investigation and the information requirements for the request to:

The PDMP staff will review the application and contact the individual who sent the request for validation. When the Board Information Request is validated, PDMP staff will query the system for the requested information and provide a report to the board.
